• Please make your check out to The Office of the Secretary of State

Submit with Payment Card (ePayment Checklist)

• A payment card may be use by completing the included ePayment Checklist form

Please Submit the original certified copy, photocopies of records will be rejected

Please submit your request form and payment together with your documents

Submitting a Request

• Expedite Pick-up times are listed at the Dropbox
• Your completed notarized documents may be submitted during your appointment

• Once completed your documents can be returned to you by mail or marked as Hold for Pick-Up
