Request A Notary Division Refund

 The online form below is used to request a refund from the Nevada Notary Division.  (Example - Notary training, Expedited, Apostille Request)

Please take the time to provide our team with as much information as possible and upload any supporting documentation to ensure timely processing. 

Before you begin, here is some information you'll need to know to correctly fill out the form:

  • Filing Type or Service you paid for
  • Confirmation or Work Order number if applicable (online filings usually will have these numbers)
  • Exact Date of payment
  • Refund Amount Requested
  • Reason for refund request
  • Payment Information (Credit Card, Cash, Check, Money Order, Trust Account)
    • Money Order number (if applicable)
    • Check Number (if applicable)
    • Last four digits of the credit card you used for this payment (if applicable)

If the original transaction is over 12 months old or you have a new card due to fraud, the refund will be in the form of a check. The 2.5% credit card fee does not qualify for a refund 

NOTE: Checks, Money Orders and Cash refunds will be mailed and you must provide mailing details in this form. Credit Card payments will be refunded to the card that was used to make the original purchase.

*Refunds can take 8-10 weeks to process.
